Fire EMS Reports

OBTAINING COPIES OF REPORTS FROM THE CITY OF HOOVER FIRE DEPARTMENT
To obtain copies of fire incident reports, please contact the Fire Administration Office at (205)  444-7655.
For copies of medical reports, due to federal regulations regarding personal health information,  the proper documentation must be completed:

• A release form from the patient or the patient's Power of Attorney, or
• Official court request with proper HIPAA release included, or
• Court Ordered Subpoena

When the proper documentation is received, the report(s) will be released to the appropriate person(s).

A downloadable copy of the release form can be downloaded here. Bring the completed form to :

The report(s) may then be released with proper identification.
